Skilled Nursing Services
A registered nurse (RN) will provide you with skilled nursing care if ordered by your doctor as part of your care plan. Your RN will also report on your progress to your doctor and instruct your family on how to help you throughout your recovery. Skilled nursing services may include medication management, catheter care, ostomy care, injections, dressing changes, wound and cast care, and nutritional counseling. Your nurse will manage and coordinate all of your services.
Physical Therapy
A registered physical therapist will evaluate your ability and implement a program to help you safely restore strength, motion and function, prevent disability, relieve pain, or adapt to a permanent disability. You and your family will be instructed in exercises, equipment use, and mobilizing techniques. With supervision from your registered physical therapist, a physical therapist assistant may provide you with some of these services.
Occupational Therapy
A registered occupational therapist or a specially trained assistant will evaluate your ability and show you ways to go about your daily tasks including self-care and leisure activities. This will help you function effectively and adapt to your home life as safely and as independently as possible.
Speech Therapy
Working with your doctor and other members of your care team, our registered speech pathologist will help you recover from your speech, language or voice disorder, or overcome any swallowing difficulties. Your therapist will provide your family members with guidance on how to support your care and rehabilitation.
Home Health Aide
Our home health aides, under the supervision of an RN or other skilled professional, will assist you in personal care and help you remain safely in an independent living setting. Services include bathing, skin care, hair care, ambulation, exercises and light housekeeping.
Medical Social Services
A home health social worker will help you and your family work through a variety of short and long range planning issues including residential care location options, financial matters, medical advocacy, and use of community resources and alternative services. A social worker is also available to help you and your family adjust to any life changes, and provide guidance on how to best support you during this time.
Who can receive Home Health Services?
Medicaid and Medicare provide complete coverage for home health services. Private insurance plans cover home health. Private payments are also accepted.
